I ONLY HAVE ONE (1) / I DON’T HAVE ANY OF THE MENTIONED VALID IDENTIFICATION CARDS?
You may use a valid POSTAL ID, COMPANY ID, or SCHOOL ID as an alternative; however, it must be supported by at least two (2) or three (3) of the following documents:
- Voter’s Affidavit from Comelec – Intramuros
- Form 137 with school dry seal
- Year book
- Social Security System (SSS) E1 form
- Old cedula / old National Bureau of Investigation (NBI) clearance
- Insurance policy
- When applicable, you must present the National Statistics Office (NSO) birth certificates from two (02) of any of your children and /or siblings
WHAT IF I HAVE NO RECORD FROM NATIONAL STATISTICS OFFICE (NSO) / THE RECORD WAS DESTROYED?
You must secure a Certificate of Destroyed or No Record from NSO and your Local Civil Registrar (LCR). For applicants born in 1949 or earlier, you must also secure a Joint Birth Affidavit of two disinterested parties with copy of their IDs.
I’M RENEWING MY OLD (GREEN) FILIPINO PASSPORT (PRE-MACHINE READABLE PASSPORT) WHAT ARE THE REQUIREMENTS?
Requirements will be the same as renewing a passport although you will have to provide original copy of your Birth Certificate from the Nationals Statistics Office (NSO).
MY PASSPORT GOT MUTILATED / DESTROYED / LOST / STOLEN. WHAT ARE THE REQUIREMENTS TO SECURE A NEW PHILIPPINE E-PASSPORT?
For Mutilated / Destroyed Passports, you need to present an Affidavit of Mutilation and all requirements stated in New Passport Applicants.
For Lost / Stolen Passports, you need to present an Affidavit of Lost and all requirements stated in New Passport Applicants. If lost passport is still valid, a police report is required as well.
